Step 1: Department Prepares Draft Amendment

The UCLA Health Office of Legal Affairs is available to review an amendment to an existing agreement.  The Department is responsible for preparing the amendment based on the terms that are desired and review of the underlying agreement.  The UCLA Health Office of Legal Affairs has created a template amendment that the Department can use as a starting point to create the amendment for the agreement.  If the only purpose of the amendment is to extend the term of an existing agreement, this template amendment can be used.

Step 2: Department Sends Amendment to Legal Affairs for Review

The Department sends the amendment to the Office of Legal Affairs when it is ready for legal review, with the underlying agreement if requested by the attorney in the Office of Legal Affairs.  The Office of Legal Affairs will review the amendment and provide legal comments as to the legal form of the amendment. 

Step 3: Department Obtains Signatures

The signature process for the amendment is the same as described for an agreement under Contract Review.
